New Patient Information

We know your time is valuable, that’s why we try to keep the new patient paperwork to a minimum. When you call to make your first appointment, the receptionist will take your personal information and have the forms completed for your signature upon your arrival. At your first visit, the nurse will collect your health history at the beginning of the exam. We’ve found our patients prefer this method as it greatly reduces the time you spend filling out paperwork. Please plan to arrive 10 minutes before your appointment time to check-in, look over and sign the necessary forms.

If you cannot keep your appointment, please call and let us know as soon as possible. If you do not show up for your appointment and do not call to let us know, that is a time slot someone else could have used and you may be charged a $25 fee.
